Job Description:

The PACE Enrollment Specialist is responsible for the initial screening and coordination of newparticipants to the San Diego PACE (Program of All Inclusive Care for the Elderly) program.He/She is also responsible for working with the PACE Marketing Manager to implement theprogram's intake, enrollment, outreach and marketing activities.1. Conducts San Diego PACE program eligibility screening, intake, education andenrollment assistance activities to initially determine participant needs andeligibility for enrollment.2. Coordinates with potential participants and/or their families to conduct initialscreenings and assessments in the homes of the potential participants.3. Completes appropriate documentation related to participant assessments asrequired.4. Coordinates communications of eligibility decisions, approvals or disapprovalswith participant or their family members.5. Participates in the interdisciplinary team (IDT), management team and otherrelated meetings and activities as needed.6. Participates in outreach and marketing activities in conjunction with the PACEMarketing Manager.7. Collects and data enters the daily referral, intake, enrollment, marketing andoutreach activities notes with submission of weekly and monthly reports to PACEMarketing Manager.8. Provide tours of PACE center(s) to interested family members, potentialparticipants, community partners and providers.9.
