Oncology Care Manager Registered Nurse Oncology - Pediatric
Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 -
16:30) Experience: 2 years of Case Manager experience, experience
with Oncology and Pediatrics, acute care experience License: RN
license required Certifications: Required - PALS or PEARS;
Preferred - CPHON Must-Have: Experience with Neuro Oncology
patients Description: This position is for a Neuro Oncology Care
Manager. The role involves coordinating follow-up appointments.
Responsibilities include ordering and following up on labs,
appointments, and scans. You will handle referrals and
authorizations. The position requires coordination of Durable
Medical Equipment (DME) ordering. Involvement in care coordination
and transition planning is expected. A needs assessment will be
conducted for each patient. Experience with Neuro Oncology patients
is a must. Knowledge of chemotherapy and its interactions is
essential, though chemotherapy administration is not performed in
this role. This position requires working within a
multidisciplinary team to develop a customized roadmap for the
patient population. You will be accountable for the assessment of
patients and the implementation of physician orders and treatment
plans. Monitoring and evaluating patient care processes and
outcomes will be part of your responsibilities. Coordination of
activities with other services and disciplines is necessary. 5
