Registered Nurse (RN) | Case Manager Location: Los Angeles, CA Agency: Lancesoft Inc Pay: $2,900 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/2/2026 About the Position The RN Case Manager coordinates and manages patient care to ensure safe, effective, and efficient outcomes. Responsibilities include assessing patient needs, developing and implementing individualized care plans, coordinating with interdisciplinary teams, facilitating discharge planning, and providing patient and family education. The RN Case Manager also monitors quality metrics, documents patient care, and ensures compliance with regulatory standards.
