Our dedicated teams work collaboratively and in partnership with family members, school administrators and faculty, county social workers, case managers, probation officers and other agencies.JOB SUMMARY:The LVN/LPT provides direct client care including medications. Maintains vigilance over the general milieu within the facility to facilitate quality client care, optimum facility operations and preserve the safety of all therein. Visually and audibly observes, documents in writing and reports orally or in writing the physical or mental condition of patients to the Registered Nurse in charge of the facility on the shift and the attending physician. Orders, prepares, administers and documents the administration of medication for clients according to physician's orders. Confirms delivery of all ordered medication(s) from pharmacy on shift worked. Responsible for writing summary on clients as assigned. Attend and participate in patient rounds, change of shift meetings, and treatment team meetings as scheduled on the assignment sheet. Makes written or oral shift assignments and plans, organizes and directs the nursing activities of the unit (when needed and assigned) to assure quality care delivery. Provides other nursing duties including first aide and medical intervention within scope of licensure as necessary for each individual client. LPT licensed staff (that meet qualifications) may be asked to become LPS designee certified to facilitate the completion of "applications 5150/5585" for clients in need of such.Bilingual (Mandarin, Cantonese, Vietnamese or Korean Languages Preferred)MINIMUM QUALIFICATIONSEducationGraduation from an accredited school of licensed vocational nursing or psychiatric technologyAssociate degree, preferred.ExperienceOne (1) year medication administration experience.License or CertificationValid California LVN or LPT license.Current CPR certification.Valid California Driver's License preferred.POTENTIAL JOB HAZARDSAssaultive clients.Blood and Body Fluid Contact (Category I)SAFETY PRECAUTIONS REQUIREDDemonstrated use of ProACT.Demonstrated use of Standard/Universal Precautions.Demonstrated use of CPR and first aid training.Demonstrated knowledge of safety procedures and Material Safety Data Sheets particular to departmental hazards (see IIPP Manual)ESSENTIAL FUNCTIONS1. Uses ProACT to appropriately intervene with assaultive/volatile clients.10. Assists in crisis intervention (including the use of ProACT) and provide support and assistance in problem resolution.11.
